ELIGIBILITY

Two winners will each receive airfare, 2 nights’ accommodation at the Westin Harbour Hotel, and an invitation to the Ontario’s Anesthesiologists Conference weekend in Toronto: September 24th-27th, 2015

 

Open to Anesthesiology Residents in Ontario


Contest begins at 12:01am EST on March 1st, 2015 and ends at 11:59 EST on May 31st, 2015.  All videos must be submitted via upload on the Ontario Anesthesiologist website, www.ontariosanesthesiologists.com/residentcontest. Winners will be selected based on the creativity of their video and impact of their answer to “What does Anesthesiology mean to you,” as determined solely by majority vote of the Ontario’s Anesthesiologist executive committee.  Winners will be notified by telephone or email by June 15th, 2015.  Winners must confirm the receipt of their prize by June 30th, 2015, otherwise the prize will be deemed void.

Important Notice

 By entering this event, participants grant the exclusive license (permission) for Ontario’s Anesthesiologist to use and edit the entry videos.  Ontario’s Anesthesiologists reserve the permanent rights to use the participants’ names and work.

By entering this contest, participants further agree that their email may be used to contact them regarding the contest and related matters, including matters related to Ontario’s Anesthesiologists generally and the Ontario Anesthesiologists Conference(s). Participants may choose to decline to receive further emails at any time or to withdraw from the contest by emailing the following address info@ontariosanesthesiologists.ca 

Ontario’s Anesthesiologists reserve the rights to withhold the distribution of prizes if the winner’s content is found to be plagiarized or if no entries are deemed to be prize worthy.

Ontario’s Anesthesiologists reserves the right to substitute a prize of equal value at its sole discretion. Ontario’s Anesthesiologists further reserves the right to select the specific airline and hotel with respect to the prize in question, and to modify this choice at any time in its sole direction.

Participants shall be liable or responsible for any legal dispute including the violation of portrait right or copyright arising from the submitted work, and shall indemnify Ontario’s Anesthesiologists and the Ontario medical Association accordingly.

A maximum of two Grand winners of the "What does Anesthesiology mean to you?" contest will be provided with airfare and 2 nights’ accommodation at hotel for the purpose of attending the Ontario’s Anesthesiologists Conference weekend in Toronto, September 24th-27th, 2015. If the winner(s) resides within ninety minutes’ driving time to Toronto, the Contest Organizers reserve the right to withhold the prize of airfare.

The grand prize does not include expenses other than the aforementioned. Ontario’s Anesthesiologists will not be held legally liable for any personal expenses incurred during the trip.

Grand prize winners must work in Ontario and be a practicing Anesthesiology Resident enrolled in one of the following Ontario Medical Schools:

  • Michael G. DeGroote School of Medicine (McMaster University)
  • Northern Ontario School of Medicine
  • University of Ottawa
  • Queen’s University
  • University of Toronto
  • Schulich School of Medicine & Dentistry (Western University).

Event prizes are non-transferable and cannot be exchanged for cash or other alternatives. The contest is not open to residents of Quebec.

Prize winners agree to release and indemnify the Ontario’s Anesthesiologist and the Ontario Medical Association regarding any dissatisfaction, loss, or damage regarding the prizes.

Ontario’s Anesthesiologist and the Ontario Medical Association will not be held liable for any direct or indirect expenses or losses associated with the event, nor any direct or indirect expenses or losses incurred as a result of participation in the event.
